疾病背景介绍
慢性淋巴细胞白血病((chronic lymphocytic leukemia,CLL), 是一种淋巴细胞克隆性增殖的肿瘤性疾病,淋巴细胞在骨髓、淋巴结、血液、脾脏、肝脏及其他器官聚集。同时它也是一种老年性疾病,以免疫功能不全的高分化淋巴细胞克隆性增殖为主要特征。而BCL2/IGH双色双融合探针可以检测染色体14q32.33处的IGH基因和18q21.33区的BCL2基因的易位。涉及的IGH(免疫球基因)和BCL2(b细胞淋巴瘤)基因的易位是CLL中位列第二的zui为常见的易位异常。
探针描述
BCL2/IGH融合基因探针,主要是以橙色荧光分子标记BCL2的DNA区域和绿色荧光分子标记IGH的DNA区域为探针,使其于癌细胞核内的BCL2和IGH基因序列杂交互补。正常情况下BCL2和IGH基因未发生融合时,在荧光显微镜下显示为两个橙色信号和两个绿色信号。当存在融合基因时,绿色和橘红色信号因重叠形成黄色融合信号。
临床意义
t(14;18)易位发生于85%的滤泡性淋巴瘤(FL)及1/3的弥漫性淋巴瘤(DL),且预后较差。IGH与BCL2相互易位是CLL中位列第二的最为常见的易位异常,可指导治疗方案的选择和预后评估。
